| Breast Augmentation Every woman has her own reasons for considering breast augmentation. Self-improvement is a natural desire. It's important to understand that changes made to the outside of a person ma effects on the inside as well. Breast augmentation isn't just a step toward changing how you look; it represents a transformation in how you feel about yourself and your body. It's important to realize that it’s unrealistic to make decisions about breast implants or breast enhancement based on magazine photos or computer-generated images. Breast Lift- Mastopexy Over the years, factors such as pregnancy, nursing, and the force of gravity take their toll on a woman's breasts. As the skin loses its elasticity, the breasts often lose their shape and firmness and begin to sag. Breast lift, or mastopexy, is a surgical procedure to raise and reshape sagging breasts, at least, for a time. (No surgery can permanently delay the effects of gravity.) Mastopexy can also reduce the size of the areola. If your breasts are small or have lost volume, for example after pregnancy, breast implants inserted in conjunction with mastopexy can increase both their firmness and their size. Mastopexy usually takes one and a half to three and a half hours. Techniques vary, but the most common procedure involves an anchor-shaped incision following the natural contour of the breast. The incision outlines the area from which breast skin will be removed and defines the new location for the nipple. Stitches are usually located around the areola, in a vertical line extending downwards from the nipple area, and along the lower crease of the breast. It is important to remember that mastopexy scars are extensive and permanent. They often remain lumpy and red for months, and then gradually become less obvious, sometimes eventually fading to thin white lines. You should also keep in mind that a breast lift won't keep you firm forever-the effects of gravity, pregnancy, aging, and weight fluctuations will eventually take their toll again. Very large, sagging breasts that restrict their activities and cause them physical discomfort trouble the majority of women who have the surgery. Some women may experience a variety of medical problems caused by the excessive weight-from back and neck pain, headaches, and skin irritation to skeletal deformities and breathing problems. Bra straps may leave indentations in their shoulders. And unusually large breasts can make a woman-or a teenage girl-feel extremely self-conscious. Breast reduction, technically known as reduction mammoplasty, is designed for such women. The procedure removes fat, glandular tissue, and skin from the breasts, making them smaller, lighter, and firmer. It can also reduce the size of the areola. The goal is to give the woman smaller breasts in proportion with the rest of her body and to help alleviate medical problems caused by the weight of the breasts. In most cases, breast reduction isn't performed until a woman's breasts are fully developed; however, it can be done earlier if large breasts are causing serious physical discomfort. The best candidates are those who are mature enough to fully understand the procedure and have realistic expectations about the results. Breast reduction is not recommended for women who intend to breast-feed. Male Breast Reduction- Gynecomastia Gynecomastia is a medical term that comes from the Greek words for "women-like breasts." Though this oddly named condition is rarely talked about, it's actually quite common. Gynecomastia affects an estimated 40 to 60 percent of men. It may affect only one breast or both. Though certain drugs and medical problems have been linked with male breast over-development, there is no known cause in the vast majority of cases. For men who feel self-conscious about their appearance, breast reduction surgery can help. The procedure removes fat and or glandular tissue from the breasts, and in extreme cases removes excess skin, resulting in a chest that is flatter, firmer, and better contoured. Surgery to correct Gynecomastia can be performed on healthy, emotionally stable men of any age. Surgery may be discouraged for obese men, or for overweight men who have not first attempted to correct the problem with exercise or weight loss. Also, individuals who drink alcohol beverages in excess or smoke marijuana are usually not considered good candidates for surgery. These drugs, along with anabolic steroids, may cause Gynecomastia. Therefore, patients are first directed to stop the use of these drugs to see if the breast fullness will diminish before surgery is considered an option. Breast Reconstruction While there are many options available in post-mastectomy reconstruction, you and your surgeon should discuss the one that's best for you. Skin expansion. The most common technique combines skin expansion and subsequent insertion of an implant. Following mastectomy, Dr. Maguire will insert a balloon expander beneath your skin and chest muscle. Through a tiny valve mechanism buried beneath the skin, he will periodically inject a salt-water solution to gradually fill the expander over several weeks or months. After the skin over the breast area has stretched enough, the expander may be removed in a second operation and a more permanent implant will be inserted. The nipple and areola are reconstructed in a subsequent procedure usually performed in the office. Some patients do not require preliminary tissue expansion before receiving an implant. For these women, Dr. Maguire will proceed with inserting an implant as the first step. Flap Reconstruction. An alternative approach to implant reconstruction involves creation of a skin flap using tissue taken from other parts of the body, such as the back, abdomen, or buttocks. In one type of flap surgery, the tissue remains attached to its original site, retaining its blood supply. The flap, consisting of the skin, fat, and muscle with its blood supply, are tunneled beneath the skin to the chest, creating a pocket for an implant or, in some cases, creating the breast mound itself, without need for an implant. Another flap technique uses tissue that is surgically removed from the abdomen, thighs, or buttocks and then transplanted to the chest by reconnecting the blood vessels to new ones in that region. Regardless of whether the tissue is tunneled beneath the skin on a pedicle or transplanted to the chest as a micro vascular flap, this type of surgery is more complex than skin expansion. Scars will be left at both the tissue donor site and at the reconstructed breast, and recovery will take longer than with an implant. Follow-up procedures. Most breast reconstruction involves a series of procedures that occur over time. Usually, the initial reconstructive operation is the most complex. Follow-up surgery may be required to replace a tissue expander with an implant or to reconstruct the nipple and the areola. Many surgeons recommend an additional operation to enlarge, reduce, or lift the natural breast to match the reconstructed breast. But keep in mind; this procedure may leave scars on an otherwise normal breast. Abdominoplasty-Tummy Tuck Abdominoplasty, known more commonly as a "tummy tuck," is a surgical procedure to remove excess skin and fat from the middle and lower abdomen and to tighten the muscles of the abdominal wall. The procedure can dramatically reduce the appearance of a protruding abdomen. But bear in mind, it does produce a permanent scar, which, depending on the extent of the original problem and the surgery required to correct it, can extend from hip to hip. The best candidates for Abdominoplasty are men or women who are in relatively good shape but are bothered by a large fat deposit or loose abdominal skin that won't respond to diet or exercise. The surgery is particularly helpful to women who, through multiple pregnancies, have stretched their abdominal muscles and skin beyond the point where they can return to normal. Loss of skin elasticity in older patients, which frequently occurs with slight obesity, can also be improved. The surgery itself takes 2 to 4 hours depending on size, and excess of skin. Dr Maguire does prefer his patients stay overnight in the hospital. You will have 3 to 4 drains held surgically in place for at least a week. The recovery time on a tummy tuck is 2 to 4 weeks for light to normal activity. Patience and listening is the key to recovery. Skin striae or stretch marks if present pre- operatively, will not disappear but are usually dramatically diminished. A simple way to test for the result of such a procedure is to widely pinch the skin just above the hips on both sides while looking in the mirror. The effect on cellulite is readily evident. Brachioplasty- Arm Lift An arm lift, also known as Brachioplasty, is a surgical procedure to remove loose skin and excess fat deposits in the upper arm. With age, upper arm skin can become loose and flabby. In some cases Dr. Maguire may suggest that liposuction be used alone or in conjunction with an arm lift to remove excess fat in the upper arms. There will be a scar that starts at the armpit and goes down the underneath part of the arm. The length will depend on body shape, size, and amount of skin to be excised. Dr. Maguire draws the excision length on the patients arm at their consultation so when they are home the patients can live with a ‘temporary scar’ see if this procedure is right for them. Brow Lift A brow lift is a procedure that restores a more youthful, refreshed look to the area above the eyes. The procedure corrects drooping brows and improves the horizontal lines and furrows that can make a person appear angry, sad or tired. In a brow lift, the muscles and tissues that cause the furrowing or drooping are removed or altered to smooth the forehead, raise the eyebrows and minimize frown lines. A brow lift is often performed in conjunction with a face lift to provide a smoother overall look to the face. Eyelid surgery (blepharoplasty) may also be performed at the same time as a brow lift, especially if a patient has significant skin overhang in the upper eyelids. Sometimes, patients who believe they need upper-eyelid surgery find that a forehead lift better meets their surgical goals. Facelift-Rhytidectomy A face lift (technically known as rhytidectomy) can't stop this aging process. What it can do is "set back the clock," improving the most visible signs of aging by removing excess fat, tightening underlying muscles, and redraping the skin of your face and neck. A face lift can be done alone, or in conjunction with other procedures such as a forehead lift, eyelid surgery, or nose reshaping. Face lifts are very individualized procedures. In your initial consultation Dr. Maguire will evaluate your face, including the skin and underlying bone, and discuss your goals for the surgery. A face lift usually takes several hours-or somewhat longer if you're having more than one procedure done. Incisions usually begin above the hairline at the temples, extend in a natural line in front of the ear (or just inside the cartilage at the front of the ear), and continue behind the earlobe to the lower scalp. In general, the surgeon separates the skin from the fat and muscle below. Fat may be trimmed or suctioned from around the neck and chin to improve the contour. Dr. Maguire then tightens the underlying muscle and membrane, pulls the skin back, and removes the excess. Stitches secure the layers of tissue and close the incisions; metal clips may be used on the scalp. After surgery a compressive garment and or dressing will be in place and must be worn at all times until Dr. Maguire removes them. Eyelid Surgery-Blepharoplasty Eyelid surgery, also known as, blepharoplasty is a procedure to remove fat usually along with excess skin and muscle from the upper and lower eyelids. Eyelid surgery can correct drooping upper lids and puffy bags below your eyes - features that make you look older and more tired than you feel, and may even interfere with your vision. However, it won't remove crow's feet or other wrinkles, eliminate dark circles under your eyes, or lift sagging eyebrows. While it can add an upper eyelid crease to Asian eyes, it will not erase evidence of your ethnic or racial heritage. Blepharoplasty can be done alone, or in conjunction with other facial surgery procedures such as a face lift or a brow lift. The recovery time from eyelid surgery is approximately 7 to 10 days. Chin Augmentation-Mentoplasty The best candidate for chin augmentation, or mentoplasty, is the individual with a receding chin and a normal dental bite. For those with a bite dysfunction, jaw surgery, in conjunction with mentoplasty, may be necessary. The procedure commonly produces a significant improvement in the profile and frequently makes the nose look smaller even when nothing is done to the nose. There are two basic procedures used in mentoplasty: one involves moving the chin bone forward and the other, the use of a plastic chin implant. A chin implant, constructed of silicone is placed deep to the soft tissue of the chin directly in front of the jawbone to create a new chin line and profile. Most of the swelling is gone within a week but subtle improvement may continue to be seen for a couple of months. A liquid diet may be advised for a day or two. Return to work and usual activity is allowed within 7 to 10 days. Within a week of surgery, the external dressing is usually removed. When treated, it can temporarily reduce moderate to severe frown lines. Botox is administered through small injections directly into the muscles causing the frown lines to relax. This treatement is minimally invasive and requires no down time. Patients will not see an immediate result with Botox alone. It does take a few days to a week to see improvement. Botox is not a filler. Using collagen fillers or Restylane in conjunction with Botox will give you a more youthful appearance. |
